What is a care manager?
Care managers are registered nurses who can help with everything from helping you understand a diagnosis given by your health care provider to educating you on lifestyle options and self-care skills for managing that condition. Care managers can help with education about a condition, preparation for doctor’s appointments, and helping you stick to the treatment plan established by your health care provider.
